1. Acknowledgment of Risk:

I, the undersigned, understand that participation in the mindfulness and wellness activities offered by Camp Heroes involves risks. These activities may include but are not limited to yoga, meditation, outdoor adventures, and group sports. While Camp Heroes takes utmost care to ensure the safety and well-being of all participants, I acknowledge that injuries, accidents, or other unforeseen events may occur.

2. Waiver of Liability:

I hereby release, waive, discharge, and covenant not to sue Camp Heroes, its officers, employees, volunteers, agents, and representatives from any and all liabilities, claims, demands, actions, and causes of actions whatsoever directly or indirectly arising out of or related to any loss, damage, injury, or death that may be sustained by my child while participating in any Camp Heroes activities or while on the premises where the activities are conducted.

3. Medical Treatment Authorization:

In the event of an emergency, I authorize Camp Heroes staff to obtain medical treatment for my child if I or the emergency contact cannot be reached or if the urgency of the situation requires immediate action. I agree to be financially responsible for any expenses incurred as a result of such treatment.

4. Media Release:

I understand that Camp Heroes may take photographs or video footage during its programs and activities. I hereby grant Camp Heroes the right to use such media featuring my child for promotional, educational, or any other purposes in any media now known or hereafter developed, without any compensation to the participant.

5. Personal Property:

I acknowledge that Camp Heroes is not responsible for personal property lost, damaged, or stolen during participation in camp activities or left on the premises.

6. Agreement to Follow Camp Rules:

I agree that my child will follow all the rules and instructions provided by Camp Heroes staff and volunteers during the course of the program. Failure to do so may result in dismissal from the program without a refund.

7. Acknowledgment:

I have read this Liability Waiver Form thoroughly and understand its contents. I am aware that this is a release of liability and a contract between myself, my child, and Camp Heroes, and I sign it of my own free will.
